How Does it Work?
After you enroll (see How Do I Join), DocuBank stores your advance medical directives and sends you your customized Emergency Card which you carry in your wallet next to your health insurance card.
If You are Hospitalized
Your Emergency Card instructs hospital staff to call 800-DOCUBANK any time, day or night. DocuBank faxes a copy of your documents promptly anywhere around the world.
DocuBank also provides the hospital with the names and phone numbers of your loved ones and primary physician so that hospital staff can reach them quickly.
Allergies and Medical Conditions can appear on the front of the card, alerting emergency
medical personnel to important care issues, while the writeable section on the back of
your card can be used for prescription information or other instructions.
Keeping your information current
DocuBank contacts you each year to ensure that your documents reflect your current wishes and to update the phone numbers of your emergency contacts.
