Established in 1994, DocuBank^® is a service of Advance Choice, Inc., providing electronic document storage and emergency access to health care documents.

With DocuBank^® hospital personnel can access health care documents (living will, health care power of attorney, organ donation information, etc.), as well as emergency contact information including names and telephone numbers of family members and primary physician, any time, day or night, around the world.

DocuBank^® memberships are available directly to individuals, as well as through attorneys, hospitals, physicians, financial advisors, CPAs, insurance brokers, trust companies, and non-profit organizations.

Advance Choice, Inc. is affiliated with the American Academy of Estate Planning Attorneys, the National Network of Estate Planning Attorneys, WealthCounsel, the National Academy of Elder Law Attorneys, and many other organizations dedicated to estate planning and elder law.
