Additional Information

DocuBank provides an optional form for additional member information as part of the S.N.A.P. service.  This form can be used to make additional medical and contact information available whenever the member's S.N.A.P. card is used. 

The Additional Information Form should be used in conjunction with all other member documents (like Healthcare Directives, Guardianship Forms, Letter of Intent, etc.).
